The Care Plan Company

Tell us what matters. We’ll turn everything someone needs to know about the person you care for into a beautiful, personalised care plan book.

Know me before you care for me.

About Me

The little things. The important things. The things that make me, me.

The Care Plan Company

The problem

Families know everything. It lives nowhere.

  • How the person communicates.
  • What causes distress, and what helps.
  • How their morning needs to run.
  • What they eat, and how.
  • How much processing time they need.
  • What good support actually looks like.

It is scattered across care plans, behaviour-support documents, staff notes and memories. When somebody new starts, the family explains everything again, and the documents that do exist are written around disability and risk rather than the person.

What we do

You tell us about them. We create the plan.

You answer short, conversational questions in your own words. Dot points and messy notes are welcome. We transform your answers into warm, first-person writing and a professionally designed printed book that introduces the person before their support needs.

“Please give me time when you’re explaining something new. Speaking calmly and allowing me time to respond helps me feel comfortable and in control.”

From a finished book, written in the person’s own voice.

Every book is anonymous by design. No names, no photographs, no clinical boxes. The personality comes from the person’s words, routines, humour and point of view.

The idea underneath

Tell us once. Keep it current. Use it wherever care happens.

Your answers become a living care profile. When something changes, you update what changed rather than starting again, and a new edition can be printed. Over time the profile becomes a record of who the person is, what matters to them, how they live and where they’re going next.
